Please complete each of the following forms. After completing and printing a form and/or viewing instructions, use your browser's back button to return to this page. Upon completion of all the forms, proceed to the next section.
Note:
AG-414: Two copies are required.
AG-401: Statement of Selective Service Registration Status - Males ages 18 through 25 ONLY.
- If you are exempt from the registration provisions of Selective Service law, please proceed to the next section after completing AG-401.
- If you are properly registered wtih the Selective Service System, verify your registration on the Selective Service Web site: https://www.sss.gov/RegVer/wfVerification.aspx and print a copy to be included with your employment paperwork.
- If you are not exempt and have not registered, you may go to the local post office to register and bring back a certificate of mailing as proof of registration or register with the U.S. Selective Service System online: http://www.sss.gov/regist.htm at this time. After registering, verify your registration on the Selective Service Web site at: https://www.sss.gov/RegVer/wfVerification.aspx and print a copy to be included with your employment paperwork. You will not be employed until your Selective Service registration has been verified.
